Case Management Services
The Safe at Home Program of Services Now for Adult Persons (SNAP) recognizes that aging persons deserve the dignity to age in place within their homes and communities. With the support of a Safe at Home Case Manager and in collaboration with other community agencies, this goal can be realized.
HOW IT WORKS
A skilled case manager conducts a comprehensive assessment to identify the client’s needs and capabilities. We then partner with the client and his/her family, as appropriate, to develop a coordinated plan. While our goal is to assist older adults to age in place, we also support the clients and their families in transitioning to a more assisted living arrangement, if needed.
SUPPORTIVE SERVICES OFFERED
Case Management offers a variety of supportive services to homebound older adults and their families. These services are provided in the comfort of a senior’s home or via telephone. These services include, but are not limited to:
- Meals on Wheels
- Supportive Counseling
- Benefit and Entitlements Counseling and Application Assistance (e.g. Food Stamps, Medicaid, SCRIE and HEAP)
- Advocacy
- Safety Calls to Clients
- Home Safety Inspection
- Referrals and linkages to community services
- Crisis Intervention
- Long Term Care Planning Assistance
- Connecting clients to our Transportation and Friendly Visiting Programs
- DFTA funded EISEP-which is a cost share based homecare program
- Supplemental funds-used to help clients in need pay for things like medical equipment, home repairs, outstanding bills
WHO IS ELIGIBLE
- Adults age 60 and over
- Adults who are no longer able to participate or attend a senior center on a regular basis
- Adults residing in Community Districts 8, 11, or 13
- Our Intake Coordinator conducts an initial assessment over the phone to determine eligibility for program services
WHAT IS THE COST OF SERVICE?
- Case Management services are funded under contract with the NYC Depart- ment for the Aging at no cost to you.
- A voluntary contribution for the meals is requested to support the Meals on Wheels program. We appreciate any contribution that our clients can afford.
- DFTA funded home care and house- keeping service fees are based on a sliding scale and may include cost- sharing for some clients.
