SNAP’S Open Door Social Adult Day Program provides quality services to individuals diagnosed with early to moderate stage Alzheimer’s or dementia. The program includes:
- Door-to-door transportation services (if needed)
- Engaging Art Classes
- Dance and Chair Exercises
- Music and Memory® Program
- Hot & Nutritious Lunch
- Snacks
- Companion Care by a Certified Home Health Aide & more
The program is held every Tuesday and Thursday, 10:00am to 2:00pm at the below location:
